Resident Transition to Care Support - Multiple location opportunities

The resident transition to care role is spending the first 6 -10 weeks supporting individual resident's as they come into our homes. Each week you would share a few hours a day am or pm once or twice a week with the resident supporting their transition with emotional support, connections and supports and information that helps them in settling into their new home. This is an important role where you foster connection with the resident and their loved ones. As you establish rapport the impact helps to reduce isolation, maintain social skills and share information and knowledge with the residents and their loved ones. You may also support the person to maintain contact with previous communities of significance and promote a sense of purpose, hence improving wellbeing.
Skills required:
- Consistent commitment and reliability
- Honesty, good communication skills
- Non-judgmental and tolerant attitude
- Ability to maintain confidential information
- Ability to work as part of a team and without constant direct supervision but under direction and in consultation with identified site supervisors
- Ability to seek assistance when required
- Ability to use initiative and problem solve and maintain boundaries
All volunteers must have the annual Influenza vaccinations, Covid vaccinations as per the ATAGI guidelines and a Police clearance to work with vulnerable persons.
