South West Community Hospice

South West Community Care (SWCC) is a local not-for-profit organisation and registered charity providing in-home aged care services across the South West. Our purpose: to help older people live safely, independently, and with dignity in the place they love – their own home. Our services follow the Wellness approach, focusing on independence, choice and enabling people to stay connected to their community. We currently support around 1,200 clients from Yarloop to Capel, offering services that maintain safety, wellbeing, and quality of life. Every service is delivered by compassionate, trained employees who work within the requirements of the Aged Care Quality Standards, whilst adhering to the Statement of Rights, and the Code of Conduct for Aged Care. Person-centred care guides everything we do, so each client receives support that best fits their life, their goals and their preferences.
